Key Aspects of Perth Home Care Packages:

Support at Home Program:

From November 2025, HCPs are merging into the new Support at Home program, focusing on simpler, more personalised care. Existing HCP clients will transition while maintaining their funding levels.

Levels of Care:

Packages are provided in four levels (Level 1-2 for low-level, Level 3-4 for high-level needs) to assist with daily living, ensuring individuals can stay at home safely.

Services Provided:

Common services include cleaning, gardening, meal preparation, medication management, allied health (physiotherapy, podiatry), and transport.

How to Access:

Access begins with an assessment by the Aged Care Assessment Team (ACAT) via My Aged Care, which determines eligibility.

Costs:

Costs depend on your financial situation and the level of care required, with government subsidies available.
